IMAGE AND VIDEO RELEASE FORM

I hereby grant permission to Studio Legale Metta, Bari, Italy, without payment or any other consideration, the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video tape. I understand that my image may be edited, copied, exhibited, published or distributed. I waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to compensation of any nature that might arise directly or indirectly from the use of my image or recording based on this release. I also understand that this material may be used in any settings, such as educational and marketing, without time, geographic or medium limitations.

For example, photographic, audio or video recordings may be used for:

• Presentations;
• Courses;
• Social Media;
• Web content and any marketing material in general.

I understand that, by selecting “yes” and hitting the “send” button at the bottom of this form, I give permission for photographic or video recordings of me to be electronically displayed also via the Internet.

I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

This release applies to any photographic, audio or video recordings collected by or provided to Studio Legale Metta.
